Registration Forms

Before you apply for online access to your medical record, there are some other things to consider. Please read the Access to GP Online Services.

Registering at the Surgery

Please complete an online registration form, or collect one from the surgery to complete and return.

If you require online access you will need to come in to the surgery with photographic I.D  and a proof of address, such a utility bill.

Information For Patients Moving Out Of Our Catchment Area

Please note that it is our practice policy to ask patients moving out of our existing catchment area to register with a practice close to your new address. We feel that your health care needs can be better met by having a local surgery which can utilize other local health services if required. You will have 30 days to register with a new GP.

Patients Living Outside the Practice Catchment Area

Front Street Surgery does not currently accept new registrations from patients who live outside our catchment area. If you live outside our catchment area and need to register with a GP, please visit the NHS website to find a surgery near you.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
