New Patient Registration
We have an open list and welcome requests for registration from patients living in or moving to our practice area.
Our practice area covers York to the west and south of the River Ouse, in particular, Acomb. We also cover Upper and Nether Poppleton, Knapton, Rufforth, Askham Bryan, Askham Richard, Bilborough, Appleton Roebuck, Acaster Malbis, Bishopthorpe, Colton, Copmanthorpe and Clifton Moor.
You can register by completing a registration form available from reception, which you will need to sign. You will also be asked to complete a questionnaire, which will provide us with some information about your medical history.
You now register with a practice rather than with a GP. When making an appointment you can request to see any of our GPs or nurses dependent upon their availability.
Information For Patients Moving Out Of Our Catchment Area:
Please note that it is our practice policy to ask patients moving out of our existing catchment area to register with a practice close to your new address. We feel that your health care needs can be better met by having a local surgery which can utilize other local health services if required. You will have 30 days to register with a new GP.
Patients Living Outside the Practice Catchment Area
Front Street Surgery does not currently accept new registrations from patients who live outside our catchment area. If you live outside our catchment area and need to register with a GP, please visit the NHS website to find a surgery near you.

Please ensure that we have your up-to-date contact details, including mobile telephone number and email address where applicable.
We send out text reminders for pre-booked appointments and may also send texts or emails on occasion for a variety of purposes, for example to invite you to a vaccination clinic, informing you about a change to the practice or asking you to update our records on your smoking status.
